What does the CFI Program mean to
potential Assisted Living residents of Summercrest?

Choices For Independence (CFI) is a program run by the Division of Human Services. It is designed to provide home care as an alternative to nursing facility care. Under the CFI program, individuals or couples 55 or older may live at Summercrest provided they meet the medial and financial criteria of this program.

If needed, services can include:

Home Health Care Services
Homemaker Services
Nursing Services
Meal Program
Personal Emergency Response System
Physical Therapy, Occupational Therapy, Speech Therapy
Medical Services, MD office visits, prescriptions, hospitalization, laboratory services, x-ray services, wheelchair transport, chiropractic and podiatrist services
