Join us at Summercrest for a free presentation on Advance Care Planning, led by Valley Regional Hospital case managers Sandra L. Boucher and Marci Thibodeau.


What Is Advance Care Planning?

Advance Care Planning allows you to document your personal beliefs, values, and wishes regarding future medical care.

In New Hampshire, this includes completing a NH Advance Directive, which consists of:

  • A Durable Power of Attorney for Health Care

  • A Living Will

During this presentation, you'll learn:

  • Why completing an Advance Directive is so important

  • What happens if you don’t have one

  • When you should complete it

  • How to properly fill it out

  • Who should have one (hint: everyone!)
