How to Become a Resident Quick Checklist

  • Contact Diane Way at (603) 863-8181 or dway@summercrest.net to schedule your tour. On this visit, Diane will tour you around the community and show you our available rooms. She will answer any questions you and your family have about Summercrest.

  • Once you complete the Reservation Agreement and General Information Form that Diane will give you, and submit a fully refundable deposit of $500.00, your unit choice may be reserved, and a tentative move-in date scheduled.

  • Now you'll need to complete the Application for Residency packet, including Financial and Medical Information forms. 

 

  • Sign and submit the Release for Medical Information Form to your health care provider. It will be extremely helpful if you contact your health care provider letting them know you are applying for residency at Summercrest Senior Living and  kindly ask them to send us the documentation in a timely manner.

  • Make an appointment with your health provider for a physician assessment. A physician assessment must be done within 30 days prior to admission for Summercrest and within 12 months for The Seasons Memory Care.

  • Our Health Services Director will make an appointment with you to complete the Resident Assessment Tool.

  • Review our Residency Agreement and Resident Handbook and write down any questions.

  • Upon financial and medical approval, set up an appointment with Diane Way to sign the Residency Agreement.  At this time the security deposit and first month’s rent are due.
